Here is a pdf link for a “2-page veterinary requirement form” that would be required to submit with us directly (not to the government). Please make sure both the medical recommendation and veterinary requirement forms are both completed prior to sending them to us.

You can submit your forms to us directly by emailing them to train@canadiancaninetraining.com or using the link below to submit them through our website. We will then create a file for you and get the process started.
